Signed and dated. Include all named entities and FEINs.
Loss runs — 3 to 5 years, valued within 90 daysRequired
Carrier-issued. Include open reserves and claim status.
Payroll by class codeRequired
Current estimated and prior year actual, split by state and code.
Experience mod worksheetHelpful
NCCI or state rating bureau worksheet, current rating year.
Currently expiring policy or declination letterHelpful
Dec page is enough. A non-renewal notice helps us position the risk.
Safety program or corrective action narrativeHelpful
Anything showing what changed after the claims. Underwriters read it.
